I could not tell you what kind of Lovecraftian space exists behind those writers room doors. The best I can say is that Nomura is similar to Kojima, or even Akira Toriyama, in that he writes in the moment. When Nomura writes things, he's rarely thinking about how that will affect things 3 games ahead or even 1 game behind. When he put Gackt into Crisis Core, he wasn't doing it because Gackt is a critical element to contextualize a famous event. He did it because he liked Gackt. Not realizing that this is now canon and it's up to the fans to make sense of it because he sure isn't.
Back to Kingdom Hearts, I can only presume that as the series continued, he was world building as he went. He'd already dropped in KH1 the concept of a world at war, that Kingdom Hearts was lost, but that children rebuilt the world's with the lights in their hearts, and that the world of KH is actually a post apocalyptic one unknown to everyone in the cast. He thought that idea was rad, which to his credit it is, and continued to iterate on that idea. Just one small problem: he never got around to making an actual timeline of events. And remember when Disney and Final Fantasy characters were supposed to be involved with this shit? So now KH1, which was more like a fairy tale or a classic Disney movie, just became a chapter in an epic story. A story no one thought about beforehand.
