The fact that Fantasy games always had injuries and deaths that could not be healed by in-universe magic? Warcraft had characters who lost limbs, characters who died to poison & disease despite dispels and cure spells existing for both of these, and in general despite ressurection being a thing, characters that canonically died and didn't come back. Oh and of course mortal wounds leading to death, despite powerful healer characters being right there.

Writing characters with missing limbs or disabilities has been extremely common in fantasy for ages.
