At least in the ORIGINAL game it wasn't looking at multiple timelines at once. It was the set future that XL wanted to change.
"Crowded hospitals, people wearing masks" doesn't make sense when at that point in the smt timeline most of humanity was gone and the remains were in underground bunkers in tokyo

The only tie in was a necklace on one character. They actually DIDN'T change a scene that COULD and probably should have been changed to more actually link with a retcon made in xenoblade 2 (related to a battle outside a space station)
And the fact they didn't change it was relatively controversial at the time of release of the remaster
