IMO the story should have been about Grimmer and Johan as two generations of whatever MKUltra project they are framing there.
There is no conflict between Johan and Tenma aside from the moral conundrum of saving the life of a psychopath that goes on to do evil deeds. There is a conga line of side characters in the story that are so much more compelling than the main story it's comical.
