The problem with evil playthroughs is that joining with main villain would usually end the plot - even if you end up usurping or fighting him from inside.
In order to have the usual structure you would need another faction of good guys you'd have to fight after dipping into evil juice. And that opens even greater can of worms. fallout New vegas did it fine, but there all factions are kind of fucked up on some level.
I kinda hate BG3 not doing the obvious evil playthrough - Joining Kethric's plan of betraying the two other dead god guys. And having Baldur's gate be just one pitched battle with you digging deeper into the city. But then again, they barely finished one act 3, so I shouldn't really ask for 2. But I will.
