it's about a nightmare world inhabited by a myriad of powerful god-like entities, the reason why they look like "porn model-tier tulpas" is because they are not human, they were made by one of the entities in his own perfect image, a lot of the monster you fight were made by other entities and some times you fight the entities themselves.
the main "plot" is about a freshly made batch of this pale humanoid creatures trying to survive in this world, they have no memory or knowledge, they don't even know how to talk, write, or even make fire, and as you progress through the game they start to learn and developed culture.
