It was an armour in the game files that had no spawn, so the patch dropped it at the dragonsblood vendor, but it was actually in the game files. People modded it in before it was even acquireable, they just couldnt get it in game because it wasnt dropped in any chests or rewards etc, an in DD2 every equipable is tied to a limited list of IDs. There are infact a lot of armours like this, like the one you wear in the flashback where you fight the dragon for the first time? Pic related. Thats an armour in the game and its the only time you see it, theres no guards wearing it or anything, its just in this flashback, so they didnt add it to the ID table and into chests/vendors, so you never see it again.

All the clothes the NPCs wear are items in the game files that are equipable, if Capcom wanted they could drop all of them at a vendor and they would all be functional if they are given unique IDs instead of requiring you replace an existing armours ID to see it.
