It's both, but it's like they're fully aware he paints himself a shade of rusty orange which is assumed to be a type of concealer he slathers on but is careful not to get in his eyes (the skin around his eyes remain a human color).
Like, "Yeah, yeah, he covers himself in makeup because he's too vain to be seen in his actual skin tone. He's 79, fat as fuck, and still considers himself sexy in spite of all evidence to the contrary. We know. Why do you keep bringing it up? You butthurt?" I guess they realize his whole persona is very low hanging comedy fruit. Like it's extremely easy to make fun of how ridiculous he is and they're like "that's all you got? pssh, we know he's a clown and we love him anyway."
