Some characters have body markings. It's less common than having wings and horns, but it's a thing. Renge has it too.

My personal headcanon is that Kivotos is some sort of afterlife limbo for girls that died too young, which is why time doesn't pass, nobody has parents and wacky things are so common. The unusual body traits like wings, horn, markings, tail...those are allegories related to how they lived and, possibly, how they died. Which would suggest that, the more unusual their bodies are, the more of an exceptional life they lived. And perhaps, the more traumatic was their death. Poor Ibuki.
