Its even sadder when you realize Gep only neglected him because he knows he's a sociopath. Its explained in a note in Rose estate that before he met his wife he was always like this, empty, all he had was being a good mechanic. When he met his wife his world gained light and joy, when she died he became even worse. He didn't want Carlo to lose the joy he had in Rose estate growing up so he just left him there, because he knows he is a dickhead. Then Carlo dies and Gep goes off the depend.

we don't know exactly how, but we know it was probably Alchemists. We know he "rushed ahead" on something and Lea was too late to save him
In a note we see Carlo was invited to go into the room where the Guardian is by the Alchemists. I believe the Guardian killed Carlo or something down there did.
