I don't think they're supposed to be so close to the cities, it's just the constraints of an open world video game. Let's say Skyrim is actually as big as these states in the US, it's not like you'd be able to see and find Boethia's shrine that easy if Windhelm was Denver and the shrine lay in the Rockies.
