The first three games in particular took a lot of inspiration from Jacob's Ladder and if you've seen that film it's not out of the realm of possibility that's what they intended for the twist to be all along, at least for the first two games there's a lot of evidence that supports that theory which was thrown out the window with 3 being a forced sequel to 1.
