this can refer to a whole range of things but the idea is that it's really bad for someone to try to lead a child who believes in Jesus/follows his example away from doing so and tried to make them sin, it's not just referring to pedos. Also the idea that there would be some kind of person literally incapable of being forgiven by Christ/God is absurd as is the idea that there would be a kind of person that are supposed to take matters into your own hands and kill/should kill themselves - both of these things are up to God, not man; the Lord wouldn't command people to kys considering suicide itself is a sin and he wouldn't tell people to kill other people (after the law of Moses has been overturned) when all of humanity is capable of being redeemed - otherwise it negates the whole point of Jesus' sacrifice and means it doesn't 100% work.
If you are a true Christian you cannot murder pedos, tell them to kys or think they are incapable of being forgiven. Only God can do that and (You) are not God. The imagery of them being thrown ino the sea and drowned with a millstone illustrates the gravity of the sin but it isn't literal, it's like saying 'leading children away from Jesus is so bad that you might as well just be drowned if you do it (and don't seek to change your ways) so don't keep doing it.'
The idea that there is an act you could commit that can permanently sever your soul from God doesn't theologically make sense in light of Jesus' sacrifice so therefore that can't be what the verse means.
