It literally is knowing that the problem exists.
That's how signs work. The sign points to a problem. If you are aware of a sign and what it means then you are aware of a problem. If you choose to ignore those signs then you are ignoring the problem.
If you aren't aware that a sign points to a problem then you aren't aware of its nature as a sign. You could not be ignoring the sign because you wouldn't even be aware that it is a sign to ignore.
This would go in the "He was really good at hiding it" box.
