I don't know if it was the worst, but it was pretty bizarre.
The second Wai Wai World fan-translation doesn't have the doc say "fuck you" anymore if you try to quit the game, but now two characters (I think Vic Viper and TwinBee, or maybe it was Konami Man and Lady) were renamed to the names of the fan-translator and their significant other.
