Mod. You can rape whoever you want, but last I checked the mod itself assumes you're only going to rape women or futanari (if enabled in options). Dark World Reborn, but you can turn off the fantasy stuff.

I want to say it was build/stature. It added a bunch of congenital genital traits and more heights/builds and injury statuses. It was far better than CK3 lewd mods which just focus on dumb shit like sex animations. Nobody cares about that, or at least I don't.
