It's not the kind of thing that can be hidden. In Spain (and all of the Hispanic countries) children have the surname of their father and their mother.
A child born out of incest would have the same surname twice, for instance: "Smith Smith." Unless there's a big coverup to illegally change the surnames of such children, this isn't a thing.
There's been official investigations to measure the prevalence of incest and the numbers are consistent with what's seen in other countries. Not to mention, if incest was truly much more prevalent than it seems to be there would be rumors about that (just like there are rumors about drug use) but there are none.
The one thing I will concede is that in most Latin American countries people don't consider fucking your cousin (especially if it's a second cousin) to be incest. They see it as kind of weird, but not inherently bad.
