I didn't say consequences don't matter or that there shouldn't be some form of punishment if you mess up, just that it's not nearly as important as intentions when judging someone's actions morally. In your example that moron will likely have ptsd and be haunted by what he did for the rest of his life and probably get jail time but there is a reason why we differentiate between 1st degree murder and manslaughter. And the punishment shouldn't be the same, obviously. This is also where "capacity to re offend" comes into play.

Yes. One of my pet peeves is when people act like they knew what you think when they have no idea. Maybe it's because I spent time with very mentally ill people I just learned that some people think in very strange ways and that reasons they do things arent always what you would guess.
