It's always a matter of opinion where the line is. For me, I would say the line for most people is when you hear something unhinged and horrific and your first inclination isn't to go "Jesus christ, how terrifying", it's "Wait, but that doesn't make sense/sounds stupid".
If you want an example, take pic-related, which is a comic that makes the claim that the average life expectancy in the Imperium of Mankind is...thirty or so years of age. Which implies death on such a massive, consistent scale for 99.99% entirety of humankind that instead of going "Oh my gosh, that sounds like a horrible place to live", it's to question how many humans even live long enough to procreate in order to suffer such massive consistent death tolls and still survive, nay outnumber the aliens trying to take down the Imperium of Mankind.
