I don't know. I live in europe and i only go to mcdonald when i'm in switzerland because i'm too poor to afford any actual restaurant or in general abroad when i need wifi and a fast meal.
I think fat people use the drive in or they pay a rider to deliver food.
Also mcdonald is one of the best fast food (at least in europe is quite good)
