For the Visitor, witnessing and changing have the same meaning, and it is capable of seeing anybody who can see it (including just through depictions of it). When you perceive it, it perceives you, and when it witnesses you, you change as a result of its observations.
Mortal minds aren't meant to inhabit the bodies that result from the absurd changes the Visitor's gaze brings, so they typically malfunction and go completely insane. A "lucky" few seem to retain their cognizance and may be able to resume a normal-ish life (depending on how drastically they mutated)
