In the manga only. The original canon version is the LN which is based on the author's real life experiences as a neet (and damn, did he admit to consuming cp and snorting coke?), quite crude. Then there's the anime based on the LN and combining some events while streamlining others.
This is one of the most controversial panels on the manga and it took lots of artistic liberties. I simply ignore it since the LN and anime indicate Misaki was truly a poor abused angel.
