The one and only thing that /v/ needs to fix its self is to just follow the only rule on the internet that matters: Don't feed the trolls.
As in, stop fucking replying to shit you don't like / don't want to see. Full stop. Just scroll past it / hide it / move on to threads that you want. Now the problem is of course that newfags will always respond to bait so this is an uphill battle but seriously the only reason any of the stuff you listed "flourishes" on /v/ is because people respond and reply to them every fucking time.
Don't respond to anything you don't want to see even if you are trying to point out the issue like "Oh, you posted this yesterday, GTFO". Don't respond to them at all and they will all go away.

In reality, it is less than 10 ~ 20 dedicated shitters who post the same threads multiple times a day, every day over and over and over and will spam any thread they don't like (this could be a current popular game or a system they don't like, etc).
Accept this fact and it becomes easy as hell to see the patterns and ignore them completely. If you have truly been here 10+ years then I question why you haven't come to this conclusion yourself.
