Lemme do a check. I can excuse it being in DT because that's an entire over-arching theme of the expansion (relationship of parents and kids). But if You look at it it only comes out strong in Pandaemonium and then the ones he did collaboratively with others. I don't remember it being an issue in 4.4, and I don't remember it being an issue in 5.5.
My guess is they just didn't have anyone else on staff since if you look at his credentials they seemed at the time to me like they were 'okay give him a shot'. They did and it was an awful disaster and Ishikawa earns some blame for having managed it and been okay with it.
