This blooper implies two things.
1.) Luanne either has really small nipples or she has ghost areolas that are the same shade as the rest of her skin.
2.) Whenever the show is slightly meta, it does imply that the Hills are portraying themselves in a dramatized version of their actual lives. Yet, Hank makes a quip about Luanne "already getting the part". This implies that she either was made up specifically for the show (i.e. Peggy doesn't actually have a niece in "real life") or Hank thought of casting someone else as Luanne, but he went ahead and let her play herself on the show.
