Your problems are, from most probable to least probable, a combination of either:
>low trial level
>bad/low level/low rarity wedges for the character + melee + gun + consonance (if it's a consonance character)
>wrong melee weapons and wrong statstick guns
>low character intron level
>not taking into account elemental advantages of certain modes (Theatre and that one type of commission I forgot the name of)
>not using your character correctly/not using her skills correctly/not using her passives correctly
