There was an instance in one of my campaigns where an NPC was a fantasy version of a real person who suffered from tuberculosis, so the explanation was that he was suffering from a debilitating curse that made him weaker and weaker and could not be cured by regular magical means. He used a cane at the start and by the end he could barely move. Finding his cure was a big part of the campaign's story.
We never even considered the combat wheelchair though. It's a horrible homebrew done by a mentally ill woman, one of those white millenial girls who thinks they're "disabled" but never specifies why or how, just rolls around in a wheelchair for some reason. It's horribly balanced, it's retarded, but WOTC saw a disabled virtue signal and fully embraced it like the retards that they are. Just like how they're embracing a fag festival in high fantasy dnd with a cute beholder enjoying the proceedings.
