The monster just stalks you for like a week or two in game, spooking you and mining around you and shit, but it doesn't attack you.
When he does attack you, it's not an instant-kill jumpscare like every other horror mod, and you can actually get him to fuck off by making a cheap wooden cross.
That and the possessed animals are pretty unsettling in their movements and shit.
Just a good spooky stalker mod like herobrine, I guess.

Yeah pretty much.
Herobrine only became a thing because of shitty Java rendering causing pixels to look like floating white eyes in cavern darkness, and some guy just said fuck it and put them on a default player skin, instead of something creative or just leaving it up to the individual's imagination.
