well, like i said, it's a holistic thing (i think)...a beboobed man is probably going to have a harder time existing as "man" in the eyes of the world around him, but i've seen a few shirtless trans men pre-op who really couldn't be mistaken for women any way you slice it. they don't need top surgery, but i'm sure it would make them more comfortable, and i believe that we should all do everything in our power to make ourselves happier and more comfortable. get top surgery or don't. i'm getting vaginoplasty, personally. i've lived as a woman for several years and my penis isn't preventing me from doing that (even when naked, because my husband sees me as a woman), but.....i don't like it very much lol. so i guess yes, it is potentially unnecessary, but i don't think at all that it is unhealthy. if it makes you happy, that is the most important thing (even if it means you pass less; i've also thought about top surgery, though i'm not really planning on getting it. i just think it could be nice)
for the second question, i guess i kind of don't know. it wouldn't make a difference to me, but i know i've heard many times men derided for being "less a man" than some other referent for having the wrong likes and dislikes. whether people mean that literally or not i'm actually not sure. so maybe? but also, i just asked my parents if they literally thought of me as less of a boy for liking feminine things as a child, and they said no. so perhaps not. maybe i'm thinking about this on too autistic a level for it to even retain any semblance of reality. maybe nothing i say means anything at all lmao. maybe that means i should reevaluate my perspective.......
