They specifically wanted TF2 Medic to be a dedicated healer and not a "combat class that can also heal" like TFC Medic. They didn't want Medics going out of their way to frag instead of healing
The syringe gun was designed as a self-defense weapon should the medic get jumped. It sucks to lead the syringes offensively but if you're being chased then it's a pretty good weapon
Also a syringe gun fits him more thematically than a shotgun, which is cool
