Each time you go to sleep in the game (it takes place over nine nights) you wake up as Fabien. You drift back and forth between the 1920s and events that led up to you opening the box in the modern day. Back in the 20s, there was a cigarette girl that Fabien, a newly turned vampire, tried to save, but she had her head torn off and metal poles staked through her. The person who killed her is back in the modern day, and has killed another woman. A girl named Pandora who wandered into a vampire bar convinced she's a vampire. She calls Fabien "Lord Fabien". When Fabien leaves the room to go check something her head has been ripped off when he gets back.

The gameplay footage shows us waking up as Fabien. This will repeat every night. The Fabien sequences were originally a standalone story DLC. But when Fabien was rewritten into a Malkavian the two characters were merged, and the plot was restructured around the parallel timelines.
