It's why the most painful things are the ones that are not painful enough for you the body to go into endorphin mode for like if you stub your toe. Also crying and rolling around in pain is a learned thing. Saying that sounds REALLY like im a tough guy but in hospitals you'll see people squel in pain and its very awkward and then other people in worse pain able to express their in a lot of pain but it hurts less for them. It's not a matter of being tougher but because of the way people are raised their reaction to pain is different.
It's very sad though because a lot of babies and children are imprinted in a way so that if a tiny noise or stumble occurs they cry because they have stupid guardians who reinforce this behaviour by going OOOOH OUUHH THATS BADD OHHH DONT CRY DONT CRYY.
It's also worth noting in the manga and the anime does it too shows time has elapsed in a way that Kaiji has been in shock and been treated so all that's left is the pain of his huge loss.
There's also a thing in animals and humans as well to a degree where the body doesn't feel pain and will remove instinctual limits on things. For example if your finger is stuck somewhere and the best thing to do is to cut it off because idk you're sliding down a mountain and there are wolves coming to get you and your finger is stuck in two boulders that YOU CANNOT MOVE OUT.
Its different for Kaiji because he's not the one pulling it on himself which makes it more scary. Also I recommend for people if you're getting something removed from your body or you're on a rollercoaster do not close your eyes becuase keeping them open and seeing whats going on will keep you a lot calmer.
