Might be just me, but I think what is rare about Lilly is that she acts like a character from a Jane Austen novel, not so much that she's empathetic.
Empathetic people aren't that rare so long as you don't scare them away.

Or rather, they tend to self-segregate into a women-only bubble.
Perhaps one of the most unrealistic parts about the whole VN is that Hanako and Lilly invite MC to their tea parties as if he were a girl. Like I get it, it happens because either of them has a crush on him, but it just wouldn't happen that smoothly and quickly even then in real life.
Then again, aren't a lot of events in literature equally unrealistic?
