Less flashy boss and enemy movesets, you have to swing an ultra class weapon 4 times in order to see the enemy flinch or stagger. You can't stagger them with a slow heavy weapon in just one hit because how will you see this epic boss combo??
Heavy weapons need to flatten and fling enemies without charge attacks again.
No more boss gap closers that can reach you from any point in their arena. Elden ring dlc was really bad with this, there's no point to creating space for a slow attack/spell when the boss can just spin and flip from any range into your face.
No more arrow/bolt cap. You already have to pay or farm in ER's case for every use of a bow or crossbow. It should at least be 999 again so you can just use them as a weapon of choice like the old games.
No more stamina cost for spells. You already have to level attunement/mind for more casts, now you have to level endurance as well? It was especially galling in Das3 where you had to raise 3 stats to end up doing less damage than just mashing r1 with a straightsword.
Allow random solo invasions again and create covenants with rewards related to invasions. The current state of ruthless ER reds is because there's no one left doing it but the most bloodthirsty minmaxers. Removing tangible rewards and meaningful soul/rune rewards means you have less people doing it just to get something besides a kill. There would be more variety and less faggotry if people who would otherwise not touch the mode would have an incentive to do so.
Smaller levels. The scale of ER is impressive until you realize they really struggle filling it with actual content. Its especially bad in the dlc where you have beautiful areas like the flower coast with nothing but scraps of crafting materials and the same skeletons and shadow undead you've fought for 100 hours already. They even made a trailer of you fighting that dancer npc in the blue fields and instead shes a faded ghost in a basement.
