I could understand him ramming the vehicle once. A diabetic with low blood sugar can lose control of their body; the In-n-Out refused to serve him so he wasn't able to get his blood sugar at an appropriate level. He was certainly fat enough to where he likely had diabetes.
But once he backed up and rammed the vehicle a second time the diabetes defense goes out the window.
