Starlight is the polar opposite of a psychopath. She cares deeply about others' feelings to the detriment of herself at times: her abandonment led to severe depression and insecurities, she thought she was unworthy to be Sunburst's friend, she thought the m6 would abandon her if she wasn't good at their hobbies, she thought Trixie would abandon her if she showed any kind of displeasure, she held extreme guilt over her past mistakes while literally having nightmares over her own perceived unworthiness of being forgiven, she was gaslit by Twilight into having a nightmare over the possibility of endangering the world over the princesses, she overworked herself trying to help as many students as possible, and she sacrifices herself for the sake of Equestria multiple times over.
Starlight is always thinking about others over personal gain, wanting to help solve others' problems, wanting to understand others on a deeper level, wanting ponies to live happily and peacefully, and her enduring philosophy is about the benefit of the whole. She made friends with all sorts of different ponies that others have trouble understanding and getting along with, like she sussed out what her chaos god friend was sperging over, and she's open to understanding and forgiving villains. Even during her days as a villain herself, the purpose of her village was to spare others from the suffering that she felt growing up. Starlight's entire character is based on empathy, a pony driven by very strong emotions and beliefs.

Starlight is the definition of social anxiety. Extroverts can absolutely suffer from it and it can greatly effect them due to the conflict between their anxiety and need to be social.
