At first Gege drew the right side of Sukuna's face like some sort of fused flesh (see pic related), but the more he drew his true form, the more it became a weird mask (it was so unclear some people thought it really was a mask made of wood). In the end he had to explain they are deformed itchy bones that have simply popped out of his face.
