Well, nothing anymore, like most of WoD.
Think it might've made them stronger body guards at the time, maybe leveled them as followers as well, but now its pretty much just for the achievement, just requires all body guard followers across both factions.
Also max love with them changes their outfit, so that's cool too I guess.

Plus I like having someone follow me around the world as well.
I miss Defender Illona.
