Main idea behind dbd is that it's a 1v4 game where 4 survivors need to repair 5 generators to power an exit gate to escape. The killer tries to kill them by hitting them twice (once to injure, and another to down) and then putting them on a meathook until they're rescued or bleed out. The killer needs to do this 3 times to each survivor to win
The toxicity mainly comes from the fact that you can do so much shit to the other side to give them a miserable experience and to win you probably should. Survivors can group with friends and have 4 coordinated people doing whatever they can to extend the game or just fuck with the killer. "Bully squads" or whatever they're called usually run flashlights (which blind the killer and stops them from picking up survivors so they can't be hooked) and will teabag whenever they can while running the killer around for ages, teabagging him, flashlighting him whenever they can. Just doing shit to make the game last forever (since there's no timelimit)
There's a ton of perks in the game that people claim are toxic to run all the time. One perk called No One Escapes Death means that when all generators are powered the killer downs you in one hit and people usually run it as a last resort and there's no way of knowing it's in play until it hits someone
Killer can also do things to completely fuck survivors. Slugging is a term where the killer downs you and leaves you there instead of hooking (since if all 4 are on hooks he wins) so if there's 2 people left he'll slug one and find the other as it makes sense to. Even when people are on the hook the killer can just camp it out because it's a reliable kill AND if he gets off the hook he can just target that guy again because nothing really stops him
