But not actually a sociopath, it's just that he can't see the world as 'real' and feels a sense of loneliness or distance from it, an unreality or fakeness from knowing or thinking that it's a fabicatedand therefore false existence.
Would be fun to see how they handle aspects of the world that they never created or addressed coming completely out of nowhere, like fairies, harpies, werewolves, different countries or factions existing outside of his knowledge, and him coming to the realization that despite creating the world he doesn't really know anything about it. The world and reality itself is a tangled web of interconnected histories and events that he'd only created a very small part of.
Also, while he can be the creator god, he shouldn't be omnipotent. I hate reading about all-powerful MCs the most, they're boring as fuck. He should be given special privileges or powers, like being able to temporarily adjust stats or erase things from memory, but nothing too overpowered. Protagonists need to struggle and suffer setbacks.
