The closest you can get to Sanae IRL in terms of both name AND function is Sanae Moreya, who is the current head of the Moreya family and descendant of the god Moreya. I am genuinely convinced that the only reason Sanae is even NAMED Sanae is because ZUN just copied the name of the real person. She's an 80 year old grandma born in 1945. See attached picture. I know that's not the answer you're looking for though.

I'd say... Imagine a plain Japanese girl, wavy black hair, but with some sort of striking feature - something like blue or green eyes. In general maybe somewhat of a rural flair, Nagano isn't very urban in the first place, and Suwa less so.
