Died to bullshit in Chapter 4. If you haven't went down a particular passage of a dungeon (to uncover it on the map) that the Glitch Girl just so happens to be down, you'll run into them and cannot escape, eventually dying to them as they can hit you several tiles away with ranged attacks. Now having to retry all fifteen floors of this level once again.
