If it's handled maturely I think it can be useful in setting up villains or showing consequences of failure in war (ie, enemy army brutalizes the populations of cities they capture). It seems silly to me that many crpgs show horrific torture and violence but are also very prudish due to real life rating systems, though I understand why.
The key phrase is handling maturely. I don't find anything about it appealing or erotic, but it does set off my protective/revenge instinct.
