This is fairly rare, and hard to completely eliminate from a pure diffusion model.

If I put my artist afro and glasses on, I'd note it's unfortunate that her groin naturally draws the eye up to the goofy legs.
Human artists fuck up anatomy too, and sometimes you want that. It's in the training set. You're going to get some incoherence, it's kind of amazing the things it is able to get right. Like gravity in picrel is mostly doing what it's supposed to.
