I am assuming you are not an all-important douche like my DM was that day and actually balance the game to not make it pure chance wether you live or die and actually let players make their own death saves AND actually let them fucking die when they got their three fails.
I am venting but my point is, the setting can be as brutal as you make it.
I think a prerequisite for this one is that everyone at the table has played and enjoyed Bloodborne. Even more than DnD. If that is the case it could be great.
Just don't expect to be using literally everything in the book. There are a few too many concepts and new rules to show of in a handful of campaigns. And I wouldn't prepare more than that, since the aforementioned specifity of the setting might lead to players getting bored of it.
If I ever DM this one, I'd make a one shot that can be expanded upon if the players so wish.
