You're right that Reimu is essentially the head priestess of her own shrine, and that's not really unusual, or well, it used to be not so unusual in the past. Shrine Maidens started out as extremely important shaman women and then gradually declined in importance until they just became janitors that know cool dance moves (not really, exaggeration). In the period that Gensokyo is stuck in, shrines operated by Shrine Maidens weren't that uncommon.
I struggle to accept that Reimu is expected to give birth. We don't know how the Hakurei clan works and whether the position of Hakurei miko is hereditary or elective. I don't think anyone would've expected a single Hakurei miko prior to Reimu, who is the first miko to operate in a Gensokyo that is bound by spell card rules, to be guaranteed to give birth. Back then, fights against youkai were lethal. The only real good argument against this is that the first Hakurei miko genuinely just looks like super Reimu.
I'm also sure that the existence of Gensokyo doesn't rest on Reimu being able or willing to give birth, either. For sure if she does, her daughter would be the new Hakurei miko, but if she doesn't, someone else will be elected to the position. As far as I know, there's nothing in Reimu's lineage that makes her able to use the Hakurei god's power or the ying yang orbs, unlike Sanae, who is only able to use miracles due to being a descendant of Suwako. If she dies and all the outside world Kochiyas (if there are any) die, then there's no one that can use the miracles of the gods of Moriya shrine.
But that's neither here nor there, this is a Sanae thread, after all, although I find Reimu interesting.

I've always liked unfiltered people. Lots of Touhou characters talk with no filter and are fairly autistic but I enjoy how Sanae forces people to listen to things she cares about without caring at all if the other side is interested or not. How many times has she talked about random video games or anime or biology or science to an audience that doesn't give a shit? She does it because she loves talking about these things, she couldn't care less if they care. It's genuine passion for it.
