I think you are confused, the 45° you're thinking of is what people refer to when using top-down today, something like Hotline Miami or the old GTA is called "true top down", old top-down or just "top perspective". When you say top-down nowadays most people think of stuff like Legend of Zelda sprites, which show the top of the head more but not completely.
