It's date triggered on server reset. From their changelogs, it's backwards compatible so you can log in days later and still get it.

I edited my birthday to be on "May 1st" and not my actual birthday (also fuck the marketing data-miners) because the update rolled out on April 30th :D so i do not know for sure. But they would not do something that's special and personal to be missable.

You get a scene with chosen character (has to be on active roster) and a personal congraturation letter and a shit song unlocks in the music Echo that's all around Rinascita.
