It's pretty alright and dare I say, fun
You might want to follow the main questline sooner rather than later because if you have the DLC that adds a new enemy, they get spawned in a lot
Some locations can be missed very easily like Necrop- I mean Underworld, though there's not much to be missed if you don't go there
There's some weird quest programming that's a bit unfinished, so I'd recommend saving regularly
The wiki is... mostly... complete on explaning both bugs and intended behaviour
Morality in game is a little weird
The vampire quest springs to mind, not completing it the way the writer wants you to was an afterthought, and you get good karma for letting a bunch of rejects terrorise the wasteland, and you don't resolve an incurable head injury that will get someone killed later. It's simply a morality check on "Do you support queers?" and not "Do you support coercive behaviour, indoctrination, degeneracy, danger, murder and cannibalism?", if it was the latter it would raise questions like why can't you side with the villain of the game.
