1. Given the final scene her mother's death feels more like a cover story at this point. With her father being potentially JSDF, it could be that they found out about Ruka being a Narby and "silenced" her mother for security reasons.
2. Seraphs can get damaged and/or break, and that one attack that she did probably broke one of her swords. But her problem could also be a mental thing given that the sword's outright not showing up instead of showing up damaged like it does with Asami. We can only wait for 5.2 for more answers.

They're all from different times but each squads are usually from the same generation. Save for a few where some are clearly from times older than the modern era, which we can only assume ended up where they were through osmosis.
